Geriatric Care
A Geriatric Care Manager is a health and human services specialist who helps families who are caring for older relatives. Our licensed clinical social worker (LCSW) specializes in geriatrics and provides the following services:
- Conduct a professional evaluation of the elderly person’s living situation and level of physical and mental functioning
- Provide expert help to clients and family caregivers in finding home care benefits, entitlements and resources

- Mediate family meetings to help family members agree on a plan of care
- Help with forms and applications for special services such as nursing home placement
- Guidance in finding a new living situation and arranging a move
